A simple tool to help an individual with self-awareness about their present mental health state, and to create an action plan for what to do to maintain good mental health, or what to do if they notice that their mental health is deteriorating.
The action plan asks people to be aware of their wellbeing by asking questions like:
What do I notice about myself?
What feelings am I having?
What activities am I doing?
What conversations I am having?
How is my diet / sleep / exercise?
It also asks about how to maintain this state, what could there be more or less of, and for a list of 5 trusted contacts for when things are really difficult.
